Glossary

Here is a glossary of terms and definitions that will help you understand important components of home health care delivery. Understanding these terms and definitions will help you use the home health service guide more effectively.

Case Management: Medicare-eligible service provided by a registered nurse, physical therapist or speech therapist which enhances continuity and appropriateness of care by providing screening, assessment, care planning, arranging for service delivery, monitoring of status and finally, evaluation and discharge.

Care Management: Care management is a term that is often used interchangeably with case management, but it may include non-medical providers.

Intermittent Care: Home care provided by a licensed professional or certified home health aide lasting up to ninety minutes, daily or multiple times per week and associated with licensed or Medicare agencies.

Supportive Care: Care at home which does not include hands-on services such as Bathing, or toileting, but is more companion focused, assisting with instrumental activities of daily living such as meal preparation, and shopping.

Custodial Care: Care for persons whose physical condition has plateaued and for whom home health services provide assistance to maintain rather than change status.

  • This type of service is NOT covered by Medicare.

Hospice Care: Palliative care for individuals at the end of life. Includes counseling, spiritual and practical support for family and caregivers together with bereavement support. Hospice represents a philosophy of caring for and supporting life to its fullest until death occurs.
